All football players and cheerleaders are to have an examination before practices start. Forms must be completely filled out by parent/legal guardian and physician. Two emergency numbers need to be supplied - people you approve to authorize medical treatment in case of injury, should be coach and/or persons who will be at or available for all games.

We will try to arrange a date and time for physicals. Details will be announced here on our website or you can contact the people whose phone numbers are listed above. Or, your personal physician can perform the physical at your convenience.

Please print and complete this Preparticipation Physical Evaluation History Form ENGLISH (PDF) SPANISH (PDF) before going to your Physical Exam.
